The Practice

Avicenna Medical Practice is an inner city Practice set in the middle of the city of Bradford. The practice serves over 7000 patients and is housed in a health centre premises with all the usual facilities for contemporary General Practice, including wheelchair access and disabled parking.

There is a comprehensive Primary Health Care Team and also many other added facilities both NHS and private.

The Practice offers a full general practice service and run specialist clinics for children and pregnant women, diabetes and asthma, Family planning clinics, Travel vaccinations, Childhood immunisations, Smoking cessation, Elderly health checks, Cervical cytology and Health services.

The practice is a local provider for Minor surgery/joint injections, Anticoagulation Services, Ultrasound, ECG, 24hour BP, Spirometry, Specialist diabetes management, and Specialist dermatology services.

The Practice operates under a Personal Medical Services contract with NHS England.

The practice is not a limited partnership.

Practice Charter

Our responsibilities

Patients have the right to be treated with courtesy by GPs, employees and other providers of health services both inside and outside the practice.

Patients have a right to confidentiality and this will be maintained at all times.

Patients should usually be seen within 20 minutes of their appointment time. Should a delay occur an explanation will be given.

In the unlikely event of a complaint, this will be acknowledged within 3 working days of the complaint. These should be put in writing and addressed to the Practice Manager. Patient suggestions and compliments are also welcome; please speak to reception who can pass on your comments.

We would like to ensure that privacy and dignity are respected. If you would like to speak confidentially to a receptionist please advise reception upon arrival.

We aim to provide a clean and safe environment. We also provide disabled access with a door bell and induction loop.

GPs and staff are trained to a high standard and undertake frequent training sessions. These take place on the third Wednesday afternoon of each month.

Your responsibilities

We expect patients to be civil to our staff at all times. Please remember they are trying to help you. We do not tolerate verbal abuse, aggressive, intimidating or violent behaviour towards our doctors, staff, patients and visitors.

Please be punctual for your appointments and inform the receptionist that you have arrived. If you no longer need your appointment, please contact reception so that it can be given to another patient.

One appointment is for one person only.

We ask patients to request home visits before 10.30 am.

Parents are responsible for their children’s behaviour whilst on the premises to ensure their safety and the right of other patients to expect a calm atmosphere.

Patients should inform the practice immediately of any change of name, address or telephone number. If patients move away from our practice area, they should register with a doctor near their new home as soon as possible, particularly if they require medication or treatment.

Dr Khan is the senior GP Principal.

He  specialises in Minor Surgery, Dermatology and Anticoagulation services as well as overseeing specialist services provided by the Practice.

He is the named accountable GP for Avicenna Medical Practice.

Dr Muhammad Javed Rehman MBChB MRCGP

GMC Number 4154848 - Male

Dr Rehman is a GP Partner at the Practice.

He specialises in teaching and training, and is a GP Appraiser. He provides specialist Diabetes services for our patients as well as those referred to our Practice by their GP.

Dr Jutta Prekow

Dr Prekow is a salaried GP specialising in sexual health.

She runs our sexual health and menopause clinics.

Dr Aamir Ali is a regular locum doctor.

Our Advanced Care Practitioners

Our Advanced Practitioners are experienced nurses who have had additional training to examine, diagnose and treat patients much in the same way as GPs. They can refer to other services and write prescriptions. A doctor is always available to consult about any problems if needed.

Gemma has an interest in women’s and infant health.

Her qualifications include degrees in Nursing and Midwifery, and a Master’s degree in Advanced Practice.

Prior to joining Avicenna in 2016 she worked for 13 years as a midwife in Leeds, and on a breast and gynaecology ward in Huddersfield for 2 years.

Our Pharmacists

Our Pharmacists issue and monitor prescriptions for our patients, ensuring they are on the correct medications and treatments. They carry out medication reviews to make sure your prescription is effective and that you are taking the right dose

Our Nurses

Practice Nurses treat patients for a wide range of common conditions (e.g. minor injuries/illnesses, removing stitches, travel vaccinations, immunisations and so on).

Our practice nurses are also specially trained to run routine clinics for certain conditions, such as diabetes and heart disease.

Each of our nurses specialises in a different area, so you need to book ahead with the most appropriate person. Our reception staff will be able to advise whether a nurse appointment is appropriate and with which nurse.

Practice and Business Manager

Our Practice Manager will be able to help you with any administrative problems you may have with the way our practice is run.

Julie will be able to help you with any administrative problems you may have with the reception team and general patient queries. She also does all the referrals for the Practice.

Reception staff

The receptionists will be the first person  who you will greet you and find out what you need. They answer the telephone; deal with enquiries, and process repeat prescriptions, and generally signpost you to the appropriate person to help you.

Administration staff

The administration staff perform a range of duties essential to the running of the Practice such as maintain record, dealing with referrals, mail maintaining clinics etc. .

Enhanced Services Administrator

The enhanced services administrator is responsible for the smooth running of the specialist clinics that are provided by the Practice such as ECGs, 24 hour BPs, Spirometry, Ultrasound, Specialist Dermatology and Diabetes Clinics.

Attached Staff

There are a number of staff who are not directly employed by the Practice but who work closely with the Practice, such as district nurses, community matrons, Health Visitors, Midwives and councillors.

District Nurses

The district nurses deal with all home nursing to patients referred by the doctor and other agencies.

They assess nursing needs and carry out treatment in the home including blood tests. They can be contacted by telephone during the following times:

Health Visitors

The health visitors are all specially trained nurses who can give advice on health care especially for expectant mothers and children under five.

The midwives look after you during your pregnancy and for two to three weeks after the delivery of your baby.

They are happy to discuss problems or answer any questions you may have.
You can contact the Barkerend team of midwives on 01274 783137 between 8.30 – 9.30am, any weekday.

Social Prescribers

They are specially trained to help our patients with all kinds of issues ranging from benefits advice, housing, and other health needs

They can be contacted via our reception team
