To register as a new patient with the Practice you must live in the practice boundary area and be able to produce documented evidence of this.

Family Doctor Service Registration forms (known as GMS1), New patient health questionnaire and ethnicity form must be completed to enable us to obtain your medical records from your current GP and to provide care safely while we wait for your records. You can download these forms from the links below.

Please hand in the forms to our Reception staff, along with the required documents for proof of identity (see below).

Please try to do this between the hours 10 am until 6.00 pm , Monday to Friday as other times are extremely busy due to patients booking appointments for the day.

Please note that it can take up to 6 for your  medical records to be sent to us. You will be able to access our services during this time but we may need information from you to provide care safely.

We also like all new patients to have a health check with the health care assistant. This helps to ensure that we understand your health needs and can provide the right care for you.

You can of course access our services if you have an urgent medical problem while we wait for your complete medical record.

Identity documents

In accordance with recommendations made by the NHS Counter Fraud and Security Management Service we request that the following details are provided by all persons wishing to register at this Surgery. Please provide us with one document from section A and one document from section B

NOTE: The same Document can not be used to confirm both Identity and Address

Section A

  • Birth Certificate:  UK Citizen.
  • Current fully signed Passport (If from a Non EU Country Passport & Visa) Visa should be longer than 6 months.
  • Current UK Photo card driving licence (Full or Provisional). UK Citizen.
  • Current EEA member state ID card
  • EU Passport
  • Photo ID plus birth certificates for children. If possible
  • Current residence permit.

Section B

  • Recent utility bill eg: Gas, Electric and Land Line Telephone – Less than 3 months old
  • Recent Bank/Building Society/Mortgage Statement – Less than 3 months old
  • Current Council tax bill
  • Current Water Rates bill

Overseas Patients

The UK has reciprocal arrangements with some countries to provide free healthcare services. We require proof of identification and a utility bill for all overseas patients.
